Formulaire de renonciation et de décharge pour le camp de hockey (SEE ENGLISH VERSION BELOW)
En contrepartie de leur participation au camp de hockey organisé par l’Association de hockey mineur de Témiscaming, les soussignés, en leur nom propre et au nom de leurs héritiers, successeurs et ayants droit, reconnaissent et acceptent par la présente ce qui suit:
1. Assomption des risques : Je comprends que la participation à des activités de hockey comporte des risques inhérents, y compris, mais sans s'y limiter, le risque de blessures graves ou de décès. J'assume volontairement tous les risques liés à ma participation au camp de hockey, y compris ceux découlant des actions ou de la négligence des autres participants, des entraîneurs, du personnel ou de tiers.
2. Autorisation médicale : J'autorise le personnel du camp de hockey à obtenir un traitement médical pour moi-même ou pour mon enfant en cas d'urgence si je ne peux être joint. Je comprends que tous les efforts seront faits pour me contacter en cas d'urgence avant qu'un traitement médical ne soit administré.
3. Décharge et renonciation à la responsabilité : Par la présente, je dégage, renonce et décharge l’Association de hockey mineur de Témiscaming, ses dirigeants, administrateurs, employés, agents et bénévoles de toute responsabilité, réclamation, demande, action et cause d'action, quelle qu'elle soit, découlant de ou liée à toute perte, tout dommage ou toute blessure, y compris la mort, que moi ou mon enfant pourrions subir en participant au camp de hockey, qu'elle soit causée par la négligence de l’Association de hockey mineur de Témiscaming ou autre.
4. Indemnisation : J'accepte d'indemniser et de dégager de toute responsabilité l’Association de hockey mineur de Témiscaming ses dirigeants, administrateurs, employés, agents et bénévoles contre toute responsabilité, réclamation, demande, action et cause d'action, quelle qu'elle soit, découlant de ma participation au camp de hockey ou liée à celle-ci.
5. Autorisation de photographier ou de filmer : J'accorde à Association de hockey mineur de Témiscaming le droit de prendre des photos et/ou des vidéos de moi ou de mon enfant pendant le camp de hockey et d'utiliser ces photos et/ou vidéos à des fins promotionnelles, y compris, mais sans s'y limiter, la publicité, les médias sociaux et le contenu du site Web.
6. Biens personnels : Je comprends que l’Association de hockey mineur de Témiscaming pas responsable de la perte ou de l'endommagement de biens personnels pouvant survenir pendant le camp de hockey.
J'ai lu le présent formulaire de renonciation et d'exonération dans son intégralité, et j'en comprends et accepte les termes. Je comprends qu'en acceptant les conditions générales, je renonce à des droits et à des recours juridiques valables.
Hockey Camp Waiver and Release Form
In consideration for being allowed to participate in the hockey camp organized by Temiscaming Minor Hockey Association, the undersigned, on behalf of themselves and their heirs, successors, and assigns, hereby acknowledge, and agree to the following:
1. Assumption of Risk: I understand that participation in hockey activities involves inherent risks, including but not limited to, the risk of serious injury or death. I voluntarily assume all risks associated with my participation in the hockey camp, including those arising from the actions or negligence of other participants, coaches, staff, or third parties.
2. Medical Authorization: I authorize the staff of the hockey camp to obtain medical treatment for myself or my child in the event of an emergency if I am unable to be reached. I understand that every effort will be made to contact me in case of emergency before medical treatment is administered.
3. Release and Waiver of Liability: I hereby release, waive, and discharge Temiscaming Minor Hockey Association, its officers, directors, employees, agents, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my child while participating in the hockey camp, whether caused by the negligence of Temiscaming Minor Hockey Association or otherwise.
4. Indemnification: I agree to indemnify and hold harmless Temiscaming Minor Hockey Association, its officers, directors, employees, agents, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to my participation in the hockey camp.
5. Photograph/Video Release: I grant Temiscaming Minor Hockey Association the right to take photographs and/or videos of me or my child during the hockey camp and to use such photographs and/or videos for promotional purposes, including but not limited to, advertising, social media, and website content.
6. Personal Property: I understand that Temiscaming Minor Hockey Association is not responsible for any loss or damage to personal property that may occur during the hockey camp.
I have read this waiver and release form in its entirety, and I understand and agree to its terms. I understand that by agreeing to the terms and conditions, I am giving up valuable legal rights and remedies.
